How Does Dr. Strawn Perform Full Correction Explant Surgery?

Dr. Strawn’s full correction explant procedure is a meticulously planned series of steps designed to create a cohesive, natural-looking result that meets each patient’s needs. While some steps may be customized according to the patient’s aesthetic preferences, the following steps are included in the full correction explant procedure Dr. Strawn developed.

Total Capsule Removal or “En Bloc” Capsulectomy

The first step is to fully remove the implants and the breast pocket scar capsule surrounding them. For patients suffering from breast implant illness (BII) symptoms, we know how important it is that every piece of the implant, scar tissue, old sutures, current and prior breast capsules are removed; therefore, our goal is to perform an en bloc capsulectomy whenever feasible, or at a minimum, total capsule removal.

The en bloc technique requires the entire capsule, including all surrounding scar tissue, meshes, sutures, or leaked silicone, to be removed in one piece. This method is essential for both silicone and saline implants, as removing the capsule in one piece avoids leakage into surrounding healthy tissues.

Sometimes, the capsule is fused to the rib. In those cases, Dr. Strawn will remove the capsule in pieces.

Pocket Irrigation

Next, Dr. Strawn will thoroughly clean the breast pocket with a triple antibiotic solution to eliminate any remaining bacteria and leave the body free of foreign substances or organisms.

Muscle Repair

Dr. Strawn will then address the pectoralis major muscle. This muscle is often severed from its origin along the sternum and lower ribs during the implant placement procedure. Dr. Strawn will separate the muscle from the capsule and the overlying breast tissue. He will then reattach the muscle and restore it to its natural state, returning function and preventing movement deformities that can result from leaving the muscle attached to the breast tissue.

This step is unique to Dr. Strawn’s full correction procedure, as he started the muscle repair trend in 2017 after seeing the results of one of his own patients. He has performed muscle repairs on all submuscular patients ever since.

Breast Lift

A breast lift, or mastopexy, can restore stretched or drooping breasts. Breast implants provide volume, but they also provide shape and support. When the implants are removed, the patient will lose both—shape and support. Mastopexies reshape and breast and combat the “pancake breast.” Patients will have varying degrees of breast ptosis, especially those who previously had large breast implants, as the implants’ weight and size can cause the breast tissue to stretch.

Mastopexies require the removal of tissue to create the most appealing aesthetic result. Dr. Strawn will keep the overall breast as large as possible during the mastopexy; however, at the end of the procedure, the size is determined by the overall tissue available.

During the breast lift procedure, Dr. Strawn removes excess skin, repositions the nipples, and reshapes the breast tissue for a more pleasing result.

Liposculpture and Fat Grafting

To enhance the aesthetic of the breasts, Dr. Strawn includes a “natural” augmentation through liposculpture and fat grafting. The patient will select an area of the body with excess fat to harvest—usually the hips, thighs, or abdomen. Dr. Strawn will use liposculpture to remove the fat while contouring the donor area for a smooth, cohesive, slimming result. He can also use liposculpture to shape the chest around the breasts, smoothing fat pockets around the underarms and creating a more aesthetically pleasing appearance.

Dr. Strawn will then isolate the fat cells before reinjecting them into the breasts. He can improve major asymmetries as well as minor imperfections in the breasts, making them rounder and perkier and increasing the volume for a beautiful, natural-looking augmentation.

What Can I Expect From Full Correction Explant Recovery?

Following your full correction explant procedure, your breasts will be securely bandaged and positioned to preserve the shape Dr. Strawn created during surgery and induce healing. You will be provided with a surgical bra to further protect your healing breasts and maintain their shape. You will receive specific aftercare instructions, including how long to wear your surgical bra and liposuction garments and when to come in for recovery check-ups.
